All new students will need to have a completed registration form, Baptismal certificate (if applicable) and birth certificate on file prior to the first day of school.

Kindergarten Students and Transferring Students:

Must be 5 years old prior to September 1st
Completed health physical form on file prior to the first day of school
Up-to-date immunization record on file in the school office
Completed dental examination form on file prior to the first day of school
Completed eye examination form on file prior to the first day of school

Second Grade:

Updated dental exam on file prior to the first day of school
